CMHA Voucher Holder Application Requirements
AT THE TIME OF APPLICATION
• $40.00 non-refundable application fee payable in cash for everyone age 18 or older.
• Copy of Driver’s License or government issued photo I.D. for everyone age 18 or older.
• Income verification (2 months of paystubs, copy of Award Letters for Social Security, SSI, Child Support, etc)
• Copy of a current water bill (must not have any delinquent amount on it and must be due two weeks or more in
the future). If it is due within two weeks, it must be paid and a receipt submitted with the water bill.
• FOR CURRENT CMHA CLIENTS: A 30 day notice must have already been submitted to your current landlord and
CMHA. You are responsible for knowing the effective date of your move and should contact CMHA to verify it. It
is extremely important you have the date correct as this could affect your ability to move forward.
• Unless previously discussed and agreed, all tenants must be prepared to move at the time the RTA and
Inspections have passed.
• A copy of the voucher (with voucher number) must be brought to the showing or emailed shortly thereafter.
The application will not be processed without it.
• Tenant MUST be eligible to transfer utilities into their name at least 72 hours prior to lease signing.
AFTER APPLICATION HAS BEEN APPROVED
• Upon approval of the application, half of the security deposit will be collected as a hold fee. This must
be paid within 3 days of an approved application.
• Prior to signing the lease, the second half of the security deposit will be collected along with the
tenant’s prorated portion of current month’s rent, if applicable. Tenant MUST have these funds
available and be ready to sign the lease within 72 hours of the passed inspection. If the tenant does not
have the funds available within 72 hours, the hold fee will be forfeit and the landlord will proceed
with a new tenant.
• If the landlord agrees to a CMHA request to lower the rent amount, the deposit listed below will be
adjusted to match the agreed upon rent amount.

• Rent is due on the 1st of every month for those with a portion of rent to pay.
• For single family homes, water bills are due on the due date indicated on the bill.

Market Rate Application Requirements

• The application must be filled out in full and legibly, including prior addresses going back at least 5
years.
• Income verification (2 months of paystubs, copy of Award Letters for Social Security, SSI, Child
Support, etc) should be brought to the showing or available to email directly after the showing.
Income must be at least 2.5x the amount of rent to qualify.
• Tenant MUST be eligible to transfer utilities into their name at least 72 hours prior to lease signing.
• Rent is due on the 1 st of every month.
• For single family homes, water bills are due on the due date indicated on the bill.
• Both must be paid on/prior to the due date every month. If either does not happen, the
tenant is eligible for eviction. ________ (please initial)
• Security deposit is equal to one month’s rent. Half of the deposit will be collected upon approval of
application as a hold fee. The balance of the deposit and the current month’s prorated rent will be due
prior to lease signing. Tenant must be prepared to move no later than the end of the current month
(unless authorization is given). If this date cannot be met, initial hold fee will be forfeit and the
landlord will proceed with a new tenant.
• Pet Policy: No pets permitted.
